Medical Claim Form

This form can help you get reimbursed for all covered medical benefits which you have already paid out of pocket. Please keep in mind when you are requesting reimbursement:

  • Clip, do not staple, all bills to the completed form.
  • Make sure all bills or itemized receipts indicate a diagnosis code, procedure code, date of service, cost, and the provider’s tax ID number.

Pharmacy Claim Form

This form is used for reimbursement of prescription drugs. If you did not present your ID card when you purchased your prescription out of pocket, you will need to submit this form for a refund. Waive Your Schools Health Insurance

Waive Your School’s Insurance

You may waive the Graduate’s student health insurance plan if you are covered by a spouse, parent, or employer under a comparable plan. Please complete the online Waiver process to determine if your current insurance is adequate and acceptable. If your current insurance coverage expires during the academic year, you have only 30 days to select new insurance under the Graduate’s student plan.

2013 – 2014 Academic Year

In order to complete the waiver, you will need the following:

  • Your current health insurance plan ID card
  • Your health insurance brochure or plan description.

Prior to beginning the waiver process, please check to make sure that your insurance meets the minimum requirements as listed below.

 

Minimum Health Insurance Requirements

In order to qualify for a waiver, your insurance must meet at least the following minimum requirements.

  • The plan provides in-patient care in the Philadelphia area.
  • The plan provides out-patient care in the Philadelphia area. Coverage for emergency-only care does not satisfy this requirement.
  • The plan provides for ancillary procedures, including laboratory tests and radiology.
  • The plan provides for outpatient and in-patient mental health care in the Philadelphia area.
  • The plan includes prescription drug coverage.
  • The total combined cost of the insurance plan’s yearly premium plus the deductible is less than $4,248.
  • This coverage will remain in force through August 31, 2014**
  • You are NOT an international student in F-1 or J-1 status.*

 

*Please note that international students, including but not limited to those in F-1 or J-1 status, may NOT qualify for a waiver. If you are NOT a US citizen or a permanent resident, you must enroll in the University’s health insurance plan unless your current plan meets or exceeds the University’s policy. We suggest you review the brochure to compare plan benefits.

 

**If your insurance will not be in effect for the entire academic year due to a life change event, you may qualify for a partial waiver during the period that your insurance will cover, and will later have to enroll in the school-sponsored insurance plan for the remainder of the academic year.

The University reserves the right to audit all waivers in order to ensure compliance with the University insurance requirements. Please be sure to retain a copy of your insurance policy as you may be asked to provide this documentation.
