Welcome to the Waive Your School’s Insurance Center!

This page is for those students who wish to opt-out (waive) out of the coverage under their school’s student insurance plan, and have the fee removed from their student account. The school’s student insurance plan can only be waived if the student has coverage under another acceptable insurance plan.

Submitting a waiver, is as easy as 123! The first step to waive out of your school’s student insurance coverage is to verify that you are a “hard waiver” student.

2013 – 2014 School Year

A hard waiver student for your school is:
A medical student.
If this is you, then you would be considered a hard waiver student. Hard waiver students are required to have health insurance to attend Marshall University.

  • Annual Waiver Deadline: 9/15/13
  • Spring/Summer Waiver Deadline: 3/2/14

A voluntary student for your school is:
A domestic undergraduate student taking 9 or more credit hours and Domestic Graduate Student taking 6 or more credit hours.
If this is you then you are exempt from the waiver process and you are not required to waive out of your school’s plan.

Welcome to the Travel Assistance Center! As a UnitedHealthcare member you are eligible for global emergency and medical assistance services provided by FrontierMEDEX. These benefits are available if you are traveling 100 miles or more from your permanent home, campus address, or while you are abroad. These services are accessible 24 hours a day, 365 days a year!

Sometimes while traveling you may feel “if it can go wrong, it will go wrong with me”. With this service, you have a caring partner to provide peace of mind for all issues that may occur while traveling. Program Summary

Worldwide Medical and Dental Referrals

We will provide referrals to help you locate appropriate treatment and quality care.

Facilitation of Hospital Admittance Payments

We will issue a prompt financial guarantee (or wire funds) up to five thousand dollars ($5,000) to facilitate admittance to a foreign (non-US) medical facility. To the extent additional funds are required; we will issue a prompt financial guarantee (or wire funds) to facilitate admittance upon securing such funds from you, your family, or your friends via wire transfer or credit card(s). You are ultimately responsible for the payment of the cost of medical care and treatment, including hospital expenses.

Dispatch of Doctors/Specialists

In an emergency where you cannot adequately be assessed by telephone for possible evacuation, or you cannot be moved and local treatment is unavailable, we will send an appropriate medical practitioner to you.

Transfer of Medical Records

Upon your consent, we will assist with the transfer of medical information and records to you or the treating physician.

Continuous Updates to Family and Home Physician

With your approval, we will provide case updates to appropriate individuals you designate in order to keep them informed.

Hotel Arrangements for Convalescence

We will assist you with the arrangement of hotel stays and room requirements before or after hospitalization.

Emergency Medical Evacuation

If you sustain an injury or suffer a sudden and unexpected illness and adequate medical treatment is not available in your current location, we will arrange and pay for a medically supervised evacuation to the nearest medical facility we determine to be capable of providing appropriate medical treatment. Your medical condition and situation must be such that, in the professional opinion of the health care provider and FrontierMEDEX, you require immediate emergency medical treatment, without which there would be a significant risk of death or serious impairment.

Transportation to Join a Hospitalized Participant

If you are traveling alone and are or will be hospitalized for more than three (3) consecutive days due to a coveredillness or injury, we will coordinate and pay for economy round-trip airfare for a person of your choice to join you.

Return of Dependent Children

If your dependent child(ren) age 18 or under are present but left unattended as a result of your injury or illness, we will coordinate and pay for one-way economy airfare to send them back to your home country. We will also arrange and pay for the services and transportation expenses of a qualified escort, if required and as determined by FrontierMEDEX.

Repatriation of Mortal Remains

If you sustain an injury or suffer a sudden and unexpected illness that results in your death, we will assist in obtaining the necessary clearances for your cremation or the return of your mortal remains. We will coordinate and pay for the expenses of the preparation and transportation of your mortal remains to your home country.

Replacement of Lost or Stolen Travel Documents

We will assist you in taking the necessary steps to replace passports, tickets, and other important travel documents.

Transfer of Funds

We will provide you with an emergency cash advance subject to us first securing funds from you or your family.

Legal Referrals

Should you require legal assistance, we will direct you to an attorney and assist you in securing a bail bond.

Message Transmittals

You may send and receive emergency messages toll-free, 24-hours a day, through our Emergency Response Center.

IMPORTANT: Insureds must call FrontierMEDEX first in order to activate this benefit. FrontierMEDEX does not reimburse monies spent for services not dispatched and rendered by FrontierMEDEX.

When calling the FrontierMEDEX Operations Center, please be prepared to provide:

1. Caller’s name, telephone and (if possible) fax number, and relationship to the patient;

2. Patient’s name, age, sex, and FrontierMEDEX ID Number as listed on your Medical ID Card;

3. Description of the patient’s condition;

4. Name, location, and telephone number of hospital, if applicable;

5. Name and telephone number of the attending physician; and

6. Information of where the physician can be immediately reached

FrontierMEDEX is not travel or medical insurance but a service provider for emergency medical assistance services. All medical costs incurred should be submitted to your health plan and are subject to the policy limits of your health coverage. All assistance services must be arranged and provided by FrontierMEDEX. Claims for reimbursement of services not provided by FrontierMEDEX will not be accepted.

(www.optumrx.com) for certain pharmacy benefit services including your mail order benefit.

The PDL includes most brand and generic medications approved by the FDA. Medications may be placed on different “tiers” based on their overall value, with Tier 1 being the most affordable tier option. In keeping with our Go Green initiative, permanent ID cards will be available by request only. Should you wish to obtain a permanent ID card, you can request one to be shipped to you by accessing your MyAccount. Permanent ID cards will be shipped 24 – 48 hours of your request.

PLEASE NOTE: If you do not enroll during your open enrollment period, you will not be able to enroll later during the 2013-2014 school year without experiencing a “life change”. This means that the loss of coverage is no fault of your own or the policyholders (i.e. you get married or become too old to be on a family plan). You will be eligible to enroll within thirty days of this “life change.” After thirty days, you will no longer be eligible.

A hard waiver student for your school is:Â

full-time student, part-time student and all international students. If this is you, then you would be considered a hard waiver student. Hard waiver students are required to have health insurance to attend University of the District of Columbia. You must complete an on-line waiver or enroll in the University’s policy, by:

Once submitted, a confirmation email is sent to your University of the District of Columbia email account immediately. Please note that it is your responsibility to save and print a copy of this email as proof of having had submitted the insurance information. If you have not received or do not receive an email, then you did not complete the entire waiver and need to resubmit it.

For those students who do not submit an approved waiver form before the deadline, you will be automatically charged and officially enrolled in the Student Injury and Sickness Insurance Plan. Once enrolled, there are no refunds or cancellations. (Please note, automatic enrollment will occur AFTER the waiver deadline.)
