Welcome to the File a Medical Claim Center! We understand the hassles that come along with filing your claims, which is why we want to make this process as quick and painless for you as possible. Below you will find all the information you will need to file a medical claim.

Submit a Claim by Mail

It’s easy! Just download the claim form below. Fill out the first page with your personal information, dates of injury or sickness, the cause of the injury or date of first symptoms for a sickness, sign your name with today’s date, and attach all receipts. Please fill out the entire first page and be sure not to leave anything blank.

Submit a Claim Electronically

Your Doctor can also submit a claim electronically, using Emedeon (formerly WebMD). This gives a faster turn-around time than submitting a claim by mail. Check with your doctor to see if they are a participant. If they are, show them your ID Card. The Emedeon information is printed on the back of every ID Card.

File A Appeal

Welcome to the File an Appeal Center! We are here to help you correctly submit your appeal, and ensure a faster turn-around time to have your claim reprocessed or reimbursement check mailed.

To file an appeal, please include the following information:

  1. Â A letter requesting an appeal to your claim(s), including your:

o  Name

o  Address

o  Phone number

o  UnitedHealthcare Student ID number

o  Date of service for your injury/sickness

o  Claim number(s) (located on the top of your Explanation of Benefits)

o  Email address

  1. A copy of your Explanation of Benefits for the claim(s) in question.
  2. Medical Records including all test results from all providers visited for the specific injury/sickness that you are appealing.

Once we receive the documentation, your appeal will be reviewed and a written response will be mailed to you. The response will include what the findings were, if the appeal was approved or denied, and the reason for the final decision.

If you have a question about your claim denial, you may call our Customer Service Department at telephone number 1-800-505-4160 for further explanation to informally resolve your appeal. If you are not satisfied with our explanation of why the claim was denied, you, or your authorized representative, may request an internal review of the claim denial. PLEASE NOTE: If you do not enroll for the year during the annual open enrollment period, you will not be able to enroll later during the 2015-2016 school year without experiencing a “Qualifying Life Event”. This means that the loss of coverage is no fault of your own or the policyholders (i.e. you get married or become too old to be on a family plan). You will be eligible to enroll within thirty days of this “Qualifying Life Event.” After thirty days, you will no longer be eligible.

VOLUNTARY DOMESTIC STUDENTS AND THEIR DEPENDENTS ENROLLMENT INSTRUCTIONS

If you are a…
an undergraduate taking 3 or more credit hours, a graduate taking 1 or more credit hours or enrolled in a departmental 601, 610 or 611 course, a non-graduate assistant/graduate fellow student, a Hershey Medical student, a graduate assistant or graduate fellow, visiting scholar, student enrolled in the intensive English communication program or student who participates in the Co-op work experience program as part of your required academic program. you are eligible to enroll online or with an enrollment form. Voluntary Students who do enroll may also insure their eligible dependents. Please see instructions below to enroll.

Waive Your School’s Insurance

Welcome to the Waive Your School’s Insurance Center!

This page is for students who wish to opt-out (waive out) of the coverage under their school’s student insurance plan, and have the fee removed from their student account. The school’s student insurance plan can only be waived if the student has coverage under another acceptable insurance plan.

Submitting a waiver, is as easy as 123! The first step to waive out of your school’s student insurance coverage is to verify that you are a “hard waiver” student.

2013-2014 School Year

IMPORTANT INFORMATION: Open Enrollment Periods for all Dependents and Students: If you have eligible Dependents in the fall or, are a student in the fall semester and eligible to purchase coverage and you choose not to enroll for coverage before the Fall Enrollment Deadline of *September 25, 2013, your Dependents or you, will not be eligible to enroll again until the start of next fall unless you experience a “Life Status” change during the year. A life status change includes marriage, divorce, birth of a child or loss of coverage through no fault of your own (i.e. aging of your parents coverage).If your Dependents or you, experience a “Life Status” change, you must submit proof of the event and enroll within 30 Days of the event, otherwise you will no longer be eligible to enroll for the remainder of the policy year.

*For new Dependents or new Part-Time students in the spring semester, your open enrollment deadline is *January 31, 2014.

Full-Time Students

If you are a…

Full-time undergraduate students taking 12 or more credit hours regardless of credit hours and Compulsory graduate students (MSW, PT, MSN, PsyD & Psy.D/JD) regardless of credit hours you are automatically enrolled unless proof of comparable coverage is furnished.  If this is you, then you would be considered a hard waiver student. Hard waiver students are required to have health insurance to attend Widener University. You must annually complete an on-line waiver or enroll in the University’s policy through your Campus Cruiser WebAdvisor.

WAIVE NOWÂ

  • Annual Waiver Deadline: 9/25/13
  • Spring/Summer Waiver Deadline: 1/31/14

After you verify that you are a hard waiver student and have other insurance, here’s what you will need:

  • Â health insurance company name, telephone number, and address
  • health insurance policy identification number
  • health insurance policy holder name and date of birth

If you complete the waiver process by the deadline, you will  be credited the insurance premium on your student statement of account. If you do not complete the online waiver process by the deadline, you will be automatically billed and enrolled for the university student health insurance plan and a non-refundable  $30 late fee  applied to your student account.

A voluntary student for your Widener University is:
Part-time undergraduate and graduate and Law School students, taking 9 or more credit hours.Â
If this is you then you are exempt from the waiver process and you are not required to waive out of your school’s plan.

Welcome to the ID Card Center! If you have lost or misplaced your ID Card, worry no more! We are here to assist you. Here, you will have instant access to a temporary ID card and have the opportunity to request a new permanent ID card from the comfort of your own computer chair.

Permanent ID Card

If you would like to request a permanent ID card or print out a temporary ID card, please click here to log into MyAccount. This allows you to request a permanent ID card that will be shipped within 24 to 48 hours of your request. While you are waiting for your Permanent ID Cards to arrive, you may also print out a temporary ID card and MyAccount will do all the work for you. This temporary ID will have your name, Policy number, and ID number already included.

Now Announcing…MyAccount Mobile Site

UnitedHealthcare Student Resources and First Student are proud to announce you can now launch our MyAccount site from our mobile version! The new site, my.uhcsr.com, puts students’ critical student insurance coverage information in the palm of their hands. On the run, no problem. You can now check the status of a claim, locate a provider or pharmacy nearby and even access your I.D. card at your convenience.

The mobile site, my.uhcsr.com , can be accessed on the iPhone 3 and 4, HTC, Samsung and Motorola Droid phones and the Blackberry v6.0. Supported browsers are Firefox, Chrome and Opera Mini. Of course, users of non-supported mobile devices and browsers can access the full MyAccount site at www.uhcsr.com.
