2016-2017 Vision Enrollment Information
 All full-time medical students and any student enrolled in a CMSRU approved alternate program of study.
Eligible students who do enroll may also insure their Dependents. Eligible Dependents are the student’s legal spouse (husband, wife or Domestic Partner) and unmarried dependent children up to age 19 or age 26 if a full-time student.

Welcome to the Waive Your School’s Insurance Center!
This page is for those students who wish to opt-out (waive) out of the coverage under their school’s student insurance plan. The school’s student insurance plan can only be waived if the student has coverage under another acceptable insurance plan.
The first step to waive out of your school’s student insurance coverage is to verify that you are an eligible student.
2016-2017 Waiver Information
Hard Waiver Student Eligibility
All registered degree seeking St. Paul Day students, Minneapolis Associate students and Certificate students who are enrolled at St. Catherine University are automatically enrolled unless proof of comparable coverage is provided.
For those students who do not submit an approved waiver form before the deadline, you will be automatically charged and officially enrolled in the Student Health Insurance Plan. Once enrolled there are no refunds or cancellations. (Please note, automatic enrollment will occur AFTER the waiver deadline.)
Please Note:Â If you waive the insurance you will not be eligible to enroll until the following school year unless you experience a Qualifying Life Event.

If you have missed the hard waiver enrollment deadline, but have not previously waived out of the insurance, you will be automatically enrolled in the insurance by your school, after the waiver deadline.
If you have previously waived out of the insurance but have since lost coverage due to a Qualifying Life Event please contact us.
Please note: If you do not enroll for the year during the fall open enrollment period, you will not be able to enroll later during the 2016-2017 school year without experiencing a “Qualifying Life Eventâ€. This means that the loss of coverage is no fault of your own or the policyholders (i.e. you get married or become too old to be on a family plan). You will be eligible to enroll within thirty days of this “Qualifying Life Eventâ€. After thirty days, you will no longer be eligible.
Eligible students who do enroll may also insure their dependents. Eligible dependents are the student’s spouse and dependent children under 26 years of age. Please see the Enrollment Instructions below.

In keeping with our Go Green initiative, permanent ID cards will be available by request only. Should you wish to obtain a permanent ID card, you can request one to be shipped to you by accessing your My Account. Permanent ID cards will be shipped 24 – 48 hours of your request.
